Youth In Transition

Youth aged 16-24 in Sudbury who have been in the care of a Children’s Aid Society or are leaving care, will work with the Youth in Transition Worker, Tom Coupal and develop a plan based on personal goals and needs.

This plan will include:

• Support with identifying, navigating, and accessing services relevant to needs
• Accessing health services such as a family doctor, housing, or counselling support
• Accessing the educational system
• Preparation for the workforce
